It Pays to be Per-Cyst-ant…

An epidermoid cyst (Epidermal Inclusion cyst, Infundibular cyst), is a benign growth commonly found in the skin and typically appears on the face, neck or trunk, but can occur anywhere on the body. Another name used is “sebacous cyst” but this is actually an antiquated misnomer, and is not a term used by dermatologists. They are also the most common type of cutaneous cysts. Epidermoid cysts result from the reproduction of epidermal cells within a confined space of the dermis. The pasty contents are mostly composed of macerated keratin (wet skin cells), which creates this “cheesy” consistency, and there can be a pungent odor. An epidermoid cyst may have no symptoms and are typically harmless. Usually people seek removal because they don’t like the appearance of these bumps, or the cyst has ruptured or been inflamed or “infected” in the past. Rupture is associated with sudden redness, pain, swelling, and local heat, and can lead to abscess formation. Also, a history of inflammation, often increases scar tissue in the area, makes the cyst more firmly adherent to surrounding skin, and makes it more difficult to remove. Surgical excision is curative, but the complete cyst removal including the entire cyst sac and contents need to be removed to ensure that the cyst won’t reoccur.

    Hello from Amsterdam, the Netherlands. My friends and I have a question. First sorry for my bad English. Question is. When you helped a patiënt from his or her cyst what happens after. I see you make sure they do not suffer during an operation. But when they go home will they have pain when the numbing wears out? Do you give them medication for home. So yes: addictive opiates. Stuff like vicodine, .I used to look house md a lot on tv. Vicodine is a medicine we do not have in Holland. I am just very curious about the pain after. With love and respect Hannah xxx

    I have watched so many of these excisions where the [physician] does not use the elliptic incision for the removal. It seems the elliptical excision makes more sense but is this a matter of common sense and training or does the physician care more about not leaving a more visible scar on the patient? Seems it would also be easier (larger opening) when removing the cyst.

I watch other Drs do this with different techniques and they are either to sloppy and their staff is unprofessional or they are too fast and act like the numbing solution is made of liquid gold. Also they have this really messy cyst but don’t check the cyst space for left over sacrifice tissue. It drives me crazy! You really are the very best.     I was wondering if she ever thinks she’s making the video of the procedure more of a priority than the patient. No disrespect intended just feel that gradually the conversation feels more for the camera & video. I get that impression when she kept asking how how hard it must of been to conceal it & how excited she must be to have it finally removed, etc. Another curiousity is prepping the area better. I had always thought that hair carries a lot of bacteria. I would have thought with an opening that large on the face it would be imperative not to have hair randomly needing to be pulled away & out of it. There could be a reasonable explanation that its not a huge concern that I’m unaware of. Again, just curious & an observation, no hate intended.?

    How is a fabric drape considered sterile? Surely a surgery like this wouldn’t be just a clean procedure.

      asuka71801 The cloth drape is sterilised and packed into sterile packaging and stored that way till it’s used. This procedure isn’t considered a sterile procedure, but that drape would have started out sterile until the patient’s hair got into the sterile field and contaminated it, rendering it no longer sterile, just clean. The fact that it’s cloth has nothing to do with its ability to be sterile or not, as surgical drapes are almost always cloth (as in the ones used during operations, not doctor’s office procedures). You’ll see a lot of paper drapes in videos, these are mainly only used in doctor’s office procedures, as they tend to fall apart if they get too wet (from the likes of blood or urine). Cloth is easily sterilised.

    I can’t believe that Americans have to walk around with cysts like this?! My GP removed a cyst from my calf area last week. He got it out whole, did internal support stiches (I mentioned that I watch this AMAZING Dr Lee whose stitching is so beautiful so perhaps he was on his best behaviour), but it cost me the equivalent of $28. And South African doctors are renowned for their excellent training. How does this happen in a first world country?

      We don’t “have to” walk around with anything. I’ve been very recently told to shut up about this already, but IMO American healthcare is the best in the world IF you have insurance and/or can afford to pay for it out of pocket. Our healthcare insurance is not government-run (except for Medicare, a federal program for people over 65, and Medicaid, which differs from state to state and covers the very poor, children, people with overwhelming healthcare costs,etc). Most people’s insurance is from a private and for-profit company (there are lots of them), and usually offered through places of employment, so it’s extremely complicated when people start making blanket comments about our healthcare. The legislators are trying to close up the insurance gap so that everyone will have at least some coverage, but for now, it’s kind of a mess! Or at least can be. Blessed are those who have insurance with a low deductible and low co-pays, it does happen, not everyone is somewhere out in the cold.

      The skin there is not any thicker than anywhere else, AFAIK. There’s a muscle layer underneath (we saw it at the bottom of the wound) and then on the inside there’s more other kinds of tissue, depending on if it’s the inside of the mouth or somewhere else. Head and neck anatomy is complicated!

      ancientsgate, they do. It takes time for them to switch hands. One rule is you never cross your hands. That blocks the view. They have to be able to clamp, cut. The same for the scrubs, too. Imagine holding a portion of the bowel. You can’t take the time to switch hands and get another hold on that slippery bowel to place a clamp on a bleeder. You waste time. When I was new, I carried a hemostat in my left pocket. During the day I worked that clamp to build up the muscles and dexterity in my left hand.

      It seems like she already partially is. She learned long ago how to use both hands, and actually if you watch her, you can see her do it all the time. Of course there is a dominant hand that she uses to clip skin with the scissors or use the scalpel or blade, but the other hand is often just as busy “helping”. It’s interesting to watch.

      Cindi Nel, you have to learn to use both hands. I did if I was assisting. It’s cumbersome and time-consuming to move a retractor to my non-dominate hand to cut suture with my right hand. Most surgeons, over time, will be adept with both hands.

      Walba Threadkiller, yvw. Glue does have it’s uses. The face, though, is where we all naturally are worried about scarring. Chances are it would work fine. Then again, if it cracks and peels, you have an angry patient.

      Walba Threadkiller, I have used glue a lot, but not on the face. After a few days, it starts to look gunky. Plus, it can start to crack and flake off. If that happens, the incision can separate resulting in a bigger scar. The sutures are going to stay there until she’s ready to remove them.

      Rhiannon Everyone has a vice in the medical field. Mine is vomit. One of my instructors couldn’t stand sputum (spit); another hated blood. The trick is to smile! It suppresses the gag reflex so you don’t puke. As long as you don’t faint at the sight of blood, it’s all good!

    It’s really interesting how you explained how to make sure the scar is as neat as possible, it’s really amazing how you sorted the little bit of puckering at the top of the incision and got it to lay flat. Probably not explained that well! I wish all took that much care with stitching I have a terrible scar on bottom inside and outside of my belly button and it was stitched in such a way there is almost no space inside it which makes it hard to clean as well as not looking very nice. Obviously it’s not as visible as the face but still upsetting. So massive thumbs up looks like you go above any beyond for you patients ?

      I know my problem is that I’m used to working under different conditions. Our patients had a hair net or cap on, too. It does seem the faces are the ones that migrate the most. You’re right that nothing is done about it. These are smaller drapes, but they could place a clamp on the pillowcase. Not too snugly, though. That could be stifling to the patient. It wouldn’t work at all if they were claustrophobic. They have to be able to see some light. But, hey, it’s entirely up to her.

      It’s odd that she doesn’t react to the slipping drape. She pokes at it (her asst. does too) and moves it around, so she IS aware of it, but that one step of securing it to something before they get started, that they don’t bother with. Oh, well, life will go on, lol!

      It seemed to me she had to either use the drape to move the hair out of the way and then get the drape resituated. She could clip the drape to the bed sheet or pillowcase. She didn’t want to touch the hair, so she used the drape to move the hair.

      I think it can be taped down to either skin or hair, but obviously Dr. Lee doesn’t think that step is important? At least I presume since she always lets the drapes float around, that it doesn’t bother her.

      Surgeons don’t use a spoon to scoop. Sorry, but I have worked with ENT, OB-GYN, orthopods, spine, general, vascular, eye, urology, plastics, none of them blindly scoop out matter. It could be extremely hazardous where this cyst was located. There are many nerves there. That’s probably why she used a q tip instead of the currette.

      many of her patients on these videos are pimple poppers themselves or live with one, so they’ve seen the videos, so they know the score. Also the reason Dr. Lee does these videos is for educational purposes, so 9 times out of 10, she’s actually saying what she does for OUR benefit, not the patients’. The patients are well-versed on why they’re being recorded before they start, so I think they’re prepared for whatever is said or done.

      Unfortunately I now have two on my face. For a while I would squeeze it all the time, but after a while I gave up. The contents are thick and without cutting i can’t get more than a small amount out and it hurts for days after, not to mention the swelling and redness. Every once in a while it gets to a point where i can squeeze a bit but I now just leave them be. 4 yrs on one and 2 yrs on the the other. Wish I could get them removed but I live in a small town and my insurance won’t cover it. Plus super expensive.

      first last I think the lady’s skin had stretched from the cyst which is why there was so much skin removed. While a small excision would have left a smaller scar, she would have been left with excess skin that looked weird. By doing it this way, Dr. Sandra was able to remove the entire cyst and sac while reducing the excess skin.

      it seems with all the moving of the drape and finally a 4×4 to keep the hair at bay that it bothers Dr. Lee. And I guess depending on the hair length and texture it probably isn’t a thing. I wouldn’t want cyst goo in my hair. And if she’s planning to send any of the sac snips to the lab, probably isn’t a good idea to have hair in it.

      I have thought about that. I may do that. I am in my second year. I had to talk to my student advisor months ago and take a leave for a while because of court, therapy for my daughter and myself, and I just can’t focus very well. Since I found out that she was abused, I have been in a fog of sorts, half mad momma bear and half mess. The guilt is debilitating and crushing at times. My therapist says that I have ptsd. I would also get horrible panic attacks which are MUCH better than they were. Anyways, I just don’t want to become in debt or mess up my credit since it’s just me. But if all else fails, I could use the financial aid once I am off leave I will look into it. Thank You for your sweet words and I wish you a Happy Holiday Season 🙂

      Jess Z I read your story and I am so sorry for what you’ve been through. Have you thought about student loans? I know it’s not the greatest thing to use student loans for purposes other than school, but since you are a full time student, the easiest access you have to a lot of money is probably student loans. Also, as long as you stay enrolled in school you don’t have to pay back the loans. You can pay it all back once you graduate and find a great job. Just a thought. I wish you continued strength. This too shall pass.

      Thank you! I’m back in school again (online degree) for healthcare administration. But I have been thinking that whenever I find a place to settle for good, and when enough time passes to where I can talk about my experience without getting upset, or whenever I feel like I’m not constantly looking over my shoulder, I’d like to become some sort of advocate. I’ve met women in shelters, and online groups like your friend…staying for way too long for whatever their reasons are. Most of them have ptsd and are ghosts of themselves. I’d like to help them. I wish there weren’t people in this world that cause others physical and emotional pain, but there are. There are tons of women in really bad places. I want to eventually become a shoulder and support system for as many as I can. THAT would be awesome! 🙂

      True, this is a clean vs sterile procedure, but this hair hasn’t been prepped like scalp hair around a pilar cyst. Why does she prep the skin and place sterile drapes? I’m just used to working under different conditions. Plus, my patients had a cap on, too.

    Every video I see has poor camera work. Why don’t they use a boom arm and a fixed focus camera with a good zoom? Camera movement up close can make people feel sick. Dr Lee has decent lighting but many don’t. A Go pro on a zoom could be inches away and perfect focus. Imagine: 4K popping?

      Brenda Buckley, as I wrote earlier, there are always conversations in the OR. If the patient is awake, we talk with them. If the patient is asleep, we talk with each other. We did surgery at least 8 hours a day, 5 days a week, plus call. We converse just as any office would.

A guy had a cyst on his scalp; after the stitches were in Dr. Lee noticed some swelling and was going to redo the stitches. I had to close the video to do something and now I can’t find it, not even in my history. Did I imagine this video?

      Danni Howard this is speculation as I’m not an expert, but I would think it is because the underlying structures in this area are more delicate and doing so would risk damaging them.

      When I was 14 a had a cyst growing on the back of my ear. It was so painful and it throbbed almost all the time. I kept trying to pop it all the time with no success but made more additional pain. 🙁 it just wouldn’t pop! My mom said it wasn’t ready. Finally at age 16. 16! The cyst was so huge that you could see it looking in the mirror! Nasty big purple bump attached to the back on my ear! It started to itch one day and finally it was able to pop by itself with only my assistants. Cyst are not exactly like pimples. Pimples are ready sooner. Cyst take their sweet time getting to a point you can pop it and deeper implanted in the skin.

      sometimes they take time to build and some dr.’s won’t touch em prematurely. ever messed with a pimple before it was ready? they only get worse. so you have to wait untill they’re ready to extract em properly to keep from coming or becoming worse. stay pretty. ✌?
